Privacy notice for the eyewear order portal.

This portal asks for more than a normal shopping site because it has to process a safety-eyewear benefit and, for RX orders, prescription glasses.

Information collected

When you place an order, we may collect your name, employee ID, store number, birthdate, phone number, email address, selected frame, order type, and order notes. For prescription orders, we also collect the prescription photo or PDF you upload, your PD, and any progressive or bifocal details needed to make the lenses.

Why those details are needed

Your employee ID and birthdate help match the order to the benefit program and reduce duplicate or incorrect orders. Your store number tells us where to ship the glasses. Your phone and personal email are used for order questions and shipping updates. Your prescription photo and PD are used only to process RX eyewear orders.

How information is used

Information is used to confirm eligibility, process RX and non-RX eyewear orders, ship orders to the correct store, contact you about order issues, maintain order records, report adoption and order activity, and improve the portal. Prescription data is not used for advertising or unrelated marketing.

Prescription uploads

Upload only the eyewear prescription needed for fulfillment. Do not upload unrelated medical records, insurance cards, lab results, or anything not needed to make the glasses. Prescription files are kept behind staff-only access controls and are not included in customer auto-reply emails.

Who may receive information

Order information may be available to CHAOSRXOptics, fulfillment partners, site administrators, and authorized business contacts who need it to process, support, or report on the eyewear program. Store and regional reporting may be summarized for program management.

SMS and email

If you provide contact information, it may be used for order-related messages such as confirmation, issue resolution, renewal eligibility reminders, and shipping updates. The order forms do not ask you to opt into unrelated marketing.

Data retention

We keep order records, including RX order details, while they are needed for fulfillment, customer support, warranty review, program reporting, legal, accounting, and operational records. In plain English: we keep the order history long enough to answer questions like what was ordered, when it shipped, whether the employee is eligible again, and whether a warranty or replacement request is valid. If you want a prescription upload reviewed for deletion after an order is complete, contact us and we will remove what we can unless we need to keep it for a legitimate order, legal, accounting, or program reason.

Security

The portal uses WordPress, Gravity Forms, and related hosting systems to collect and store order information. No system is perfect, so access should be limited to people who need the information to operate the benefit program.
